Root Cause Analysis (RCA)

The key to preventing recurrence of an undesired event (i.e., equipment failure) is to understand the true root cause of that event and to take steps to eliminate the root cause. This sounds like commons sense, and it is. Problems come into play when human bias, emotion, and lack of data come into play. We firmly believe that Root Cause Analysis, like most other reliability analyses, is not an activity that you can outsource completely. We can however provide analysts to facilitate the Root Cause Investigation using a widely accepted Cause Mapping approach. Facilitation by a third party is an invaluable tool in Root Cause Analysis and goes a long way to ensure objectivity.
